For inpatient hospice referrals:
Referrals can be initiated by any medical doctor who is caring for the patient. Referrals from restructured hospitals, community hospitals and some homecare services are made through Agency of Integrated Care portal. Alternatively referrals can be made by filling in referral forms which can be downloaded from Singapore Hospice Council website. There are fax numbers on the hard copy of the forms. Patients may be admitted from homes, hospitals or nursing homes.
The referring doctor has to determine if patients meet the admission criteria of having a life limiting illness with prognosis of less than 3 months and that patients are not able to be cared for at home due to symptoms or nursing needs.
Families are welcome to make an appointment to visit the hospice to view our premises and facilities before making any decision.
After receiving the hospice referral form from the patient’s doctor and deeming the admission appropriate, we will liaise with the hospital on bed availability and patient transfer.
During the transfer, a family member will be notified to accompany the patient and help with the registration.
Upon arrival at Dover Park Hospice, the patient will be settled into his bed and assessed by our nurses and the admitting doctor. Our team will meet with the family to understand more fully, the needs and concerns of the patient and the family.
