Living with a life-limiting illness can sometimes feel isolating, especially when spending long periods of time at home.

Day care provides a safe and supportive environment where patients can receive care, participate in meaningful activities and spend time with others who share similar experiences.

At our Day Care centre, patients are supported by a multidisciplinary team that focuses on maintaining comfort, function and quality of life.

Our team work with each patient to understand their needs so that care is personalised and meaningful.

Who We Serve

We care for clients with life-limiting illnesses, such as those: 

  • With advanced terminal-illness (cancer or non-cancer), with an estimated prognosis of less than one year
  • Whose symptoms are stable and are functionally able to tolerate sitting and travelling to Day Care
  • With physical and psychosocial care needs that require day time support, monitoring and maintenance, including those isolated at home due to advanced illnesses who wish for community and social interaction and to participate in meaningful activities
  • With chronic advanced lung diseases enrolled in R.I.S.E. (Restitutive. Integrative. Supportive. Empowering) Programme.
    • Our curated 12-week programme empowers patients with skills and confidence to self-manage breathlessness on their own in the community through symptom monitoring, physical rehabilitation, self-awareness and confidence building, education and psychosocial support for behavioural change.

Rehabilitation Activities

Alongside daily activities, our rehabilitation therapies bring comfort and calm to patients through: 

  • Physiotherapy and Occupational Therapy: Gentle, tailored exercises that ease physical discomfort, preserve mobility and encourage independence in daily living
  • Relaxation and Sensory Therapy: Calming massage and aromatherapy that relieve tension, ease symptoms and nurture overall wellbeing
  • Group Sessions: Shared activities such as culinary group sessions, crafts and games that help evoke memories and foster a sense of belonging

R.I.S.E (Restitutive. Integrative. Supportive. Empowering) Programme

Patients with chronic advanced lung diseases

In collaboration with Tan Tock Seng Hospital and Ren Ci Hospital, the R.I.S.E Programme is a 12-week programme that equips individuals with chronic lung diseases with the skills and abilities to self-manage their breathlessness at home.

Through a multidisciplinary team comprising a doctor, nurses, medical social worker, occupational therapist and physiotherapist at Dover Park Day Care, the programme provides nursing, psychosocial and rehabilitation care to optimise physical functions through functional pulmonary rehabilitation therapy. Thus, enhancing their quality of life and allowing reintegration to communities.

Frequently Asked Questions

1. What are the referrals and admission process?

Patients should approach their primary doctor from hospital or healthcare institutions to submit a medical referral. The referring doctor will assess whether the patient meets admission criteria. Our medical team will then review the referral and speak with your primary doctor on the care plan.

Upon acceptance of referral, a call will be made to the referred patient and family, for further clarification on the service provided and the necessary arrangements to be made for the first visit.

2. What are the charges and subsidies?

Our Day Care Service is funded at no charge (including transport), excluding ad-hoc consumables which can be claimed through MediSave, subject to prevailing policies.

Patients may also utilise MediSave from approved dependents (spouse, children, parents, grandchildren, and siblings).

Our Patient Services team will guide you or your loved ones through the estimated fees and available support and answer any questions you might have.

3. What are your operating hours?

Monday to Friday: 10.00am to 3.00pm

4. Is transport provided?

Yes, 2-way wheelchair transport is provided at pick-up/drop-off point

5. Are meals provided?

Yes, breakfast, lunch and tea break are provided.

6. Can I get help with Advanced Care Planning?

Yes. We will guide the patient and their caregiver in discussing care preferences and making informed decisions for the future.
